Registration Form
The application below represents a request for admission. It is not binding upon the applicant or John Boner Neighborhood Centers. Should John Boner Neighborhood Centers be able to accept the student, a place will be reserved. Registered youth who cannot be enrolled immediately will be placed on a waiting list and be contacted by EDGE Program Staff as soon as a space opens.

Additional Steps for EDGE Enrollment
John Boner Neighborhood Centers collects information on its program participants not only to comply with funder requirements, but to track the effectiveness and improve the quality of our programs. Similarly, we have developed standards of conduct to ensure that our programs operate efficiently and that both our program participants and our staff remain safe. To that end, before your child can attend the program, John Boner Neighborhood Centers' EDGE Program requires completion of the IndyEast Promise Neighborhood Consent and Enrollment form and the EDGE Parent Packet.

IndyEast Promise Neighborhood Consent and Enrollment Form
IndyEast Promise Neighborhood (IEPN) is a collaborative initiative launched in 2023 by John Boner Neighborhood Centers (JBNC) and Edna Martin Christian Center (EMCC) encompassing the Near Eastside and Martindale Brightwood communities. The program provides services to children and families within the neighborhood, aiming to improve educational and developmental outcomes and to ensure that every child in the IndyEast Promise Neighborhood will be ready for college and prepared for a career.

To that end, IEPN has partnered with additional community organizations to perform research related to the initiative’s outcomes to evaluate progress toward achieving its goals. While you are not required to participate in the research, you must complete the online form once your child has been offered a spot in JBNC’s EDGE After School Program. The consent and enrollment form will ask whether you agree to participate in providing information to our community partners, as well as collecting necessary information for your child to enroll in JBNC’s EDGE After School Program, along with other services if you so choose. As an EDGE Program participant, your input will be beneficial to improving services and ensuring that IEPN is able to fulfill its promise.

EDGE Parent Packet
The EDGE Parent Packet includes the collection of:

• Student Health Information (to include: accommodations, allergens, medications, medical authorization, and immunization records)
• Authorized Pick-Up Person and Additional Emergency Contact Information

and consent to the following policies:

• Indiana Public Schools' Data Sharing Agreement
• Field Trip Permission
• Photo and Video Release
• EDGE Attendance Policy
• EDGE Behavior Management Policy
• EDGE Transportation Policy 

The link to these forms and agreements will be available for you only after your child has been offered a space in the program. If you choose not to complete the forms at that time, your child will not be eligible for enrollment in the EDGE Program. Applicants will be allowed to attend the program only after all forms have been completed and returned.
